This Brand New AC Adaptor is exactly as shown in the Photos Below.

Output = DC 12V 10A

You simply Wire in your Mains Lead into L N and Earth and then you have 2 Outputs with a Voltage Adjustment Screw at the very end.
 
Inside the PSU is a switch to switch between 110V & 220V.
